The Mid America Chapter strives to meet the needs of individuals living with multiple sclerosis in a variety of ways including access to financial guidance, referral resources, and possible financial assistance.

Eligibility

Anyone with a confirmed diagnosis of MS is eligible to apply. There are no income qualifications for this program and each case is assessed on a case by case basis.

  • Utilities
  • Rent / Mortgage
  • Phone
  • Transportation Services
  • Respite Care / Home Care
  • Home Modifications
  • Medical Supplies / Equipment
  • Mental Health Services
  • Vehicle Modifications
  • Dental Assistance
  • Medical Bills
  • Air Conditioner Repair / Install
Ineligible items include:
  • Cable
  • School Tuition
  • Credit Card Payments
  • Prescription Drugs
  • Car Payments
  • Court Fees / Tickets
  • Taxes
  • Painting / Guttering
  • Car Registration
  • Legal Fees
  • Insurance

Application Process

In order to apply for financial assistance from the National MS Society - Indiana State Chapter, individuals must provide the following items:
  • Completed and signed application

  • Documentation of the cost of the item or service for which you are seeking assistance
  • Physician’s prescription or other supporting documents if this is a medically-related item or service
  • Confirmation of your diagnosis of MS, if you have not previously provided this to the Society:

The Mid America Chapter's Financial Assistance Committee reviews all completed financial assistance applications the first Tuesday of the month and all paperwork must be tunred in the Monday before the committee meeting in order to be considered for funding.
