Case management is designed to assist people with the highest and most complex level of needs (such as dually diagnosed clients living with MS and a mental health disorder, or clients living with progressive MS).
A case manager will develop a plan of action, coordinate among multiple professionals and agencies, advocate on behalf of the client to affect the action plan, document activities and determine the appropriate time for case closure and/or termination of services.
