We are planning to recognize Indiana’s MS Superstars at the Indiana State Chapter’s Annual Meeting on November 14, 2009. We are seeking your nominations for excellence in the following categories:
• Neurologist/Doctor
• Nurse
• Clinic/Medical Practice
• Infusion Site
• Employer
• Service Provider/Community Agency
