Adaptive equipment and home or vehicle modifications can be essential to helping people with MS live independently. This grant assists with the purchase of items such as walking aids, bath safety, scooters, ramps, cooling vests, air conditioners, service dogs and more.
Eligibility
To be eligible for an Adaptive Equipment/Home and Vehicle Modification Grant:
- You must be registered with the National MS Society, Minnesota Chapter. If you are not already registered, visit the homepage and click “Create an account.”
- You must have a doctor’s prescription for the item or modification.
- The item or modification must be covered under the chapter’s Adaptive Equipment/Home and Vehicle Modification Grant.
How it works
- Before making a purchase, first contact the chapter office for authorization. You can then contact the vendor of your choice or ask chapter staff for a referral of preferred vendors in your area.
- All independent living aid requests will be subsidized according to the total amount of the balance after all third-party payments have been made. Third party payments include: private insurance, HMOs, Medicare, rehabilitation services and any donation by another individual and/or organization.
- The amount the Minnesota Chapter will reimburse per request is based on a payment-percentage sliding fee scale (see chart below). The balance of each item will then be the responsibility of the registrant.
- Reimbursement begins at $25.
- The maximum amount of reimbursement per person, per fiscal year (Oct. 1 to Sept. 30) is $1,500.
New items covered under this program include: wheelchairs, walking aids, bath safety, scooters, patient lifts, leg braces, hospital beds, wheelchair and power mobility parts, seating systems, portable and fixed ramps, home modifications, lift chairs, stair glides, vehicle modifications, standing frames, cooling vests, air conditioners, service dogs, exercise equipment, urological and incontinent supplies.
