Sometimes, MS can make it difficult to keep up with daily chores and property maintenance. The Chore Services Grant provides limited financial assistance to help people with MS pay for daily chore services, property maintenance and more. This program can help people with MS remain independent in their own homes for a longer period.
The National MS Society, Upper Midwest Chapter can provide limited financial assistance to help purchase chore services for any person with multiple sclerosis who’s registered with the chapter.
Funding guidelines
- The Chore Services Grant helps people with MS remain independent in their own homes and is meant to provide assistance with household chores, property maintenance and other daily routines.
- Eligible services for reimbursement include small household chores, meal preparation, laundry assistance, routine yard maintenance, snow removal, housekeeping/cleaning, and running errands.
- Alternative resources for reimbursement (ie: Medicare, Medical Assistance/Medicaid, private insurance, and/or state, county or private funding) must be explored before applying for the chapter’s grant.
- Chore services must be provided by a licensed or insured agency and payment for services will be made directly to the agency providing services. Agencies must complete and return a W-9 tax form to the chapter before payment can be made.
- The grant can assist with a maximum of $750 per person, per fiscal year. Fiscal years run Oct. 1 through Sept. 30.
- The Chore Services Grant is a first come, first-served process based on available funds.
Application process
- Contact the Upper Midwest Chapter at 800-582-5296 and press “1” to request application materials and/or referrals for chore service providers in you area.
- Complete the application and send it to the National MS Society, 200 12th Ave. S., Minneapolis, MN 55415, along with written confirmation of MS diagnosis and a prescription stating need for chore services by your physician.
