When a person with MS needs transportation, eligibility for assistance must be determined, the individual must be registered with the National Multiple Sclerosis Society, Gateway Area Chapter, a confirmation of their diagnosis must be on file, and have an up-to-date financial assistance form completed with each request.
Transportation provided for the following appointments and programs:
- Physician visits – neurologists, primary care physicians
- Physical, Occupational and Speech/language therapy
- Education programs
- Family programs
- Wellness programs
Referrals to other community resources will be made as appropriate to ensure access to quality medical and psychosocial care.
