Reach Out: Services to Maintain Independence

Care management has become a priority of the National Multiple Sclerosis Society. The goal is to promote enhanced quality of life and help people with MS achieve the highest level of independence and autonomy. Families need to know that the Society has made a commitment to help them deal with the complexity of accessing care and services, and will advocate on their behalf to secure needed interventions.

People living with MS and their care partners sometimes face multiple complex issues. Housing, transportation, medical, financial and insurance problems can be overwhelming when living with a chronic disease. This program offers persons living with MS,  who are experiencing multiple issues, planned short-term social work services that will enable the client to maintain their independence and improve their quality of life. Services provided through Reach Out: SMI are designed to solve current problems and promote on-going (long-term) stability. To find out if you qualify for this program, call 1-800-344-4867.
