Continuing Care Outreach Program
People with MS in long-term care facilities deserve the best care available from educated CNA staff who can understand the person as well as the disease. This was our goal as we designed this innovative cutting-edge program. This program utilizes an all-volunteer trained staff, who contact long-term care facilities and offer our continuum of care services thereby maximizing independence and improving quality of life for those living with MS.
The program goals are:
• To introduce a continuum of free services from the National MS Society, Mid-Atlantic Chapter to facility staff and residents of long-term care facilities.
• To identify residents with MS living in LTC facilities who might benefit from NMSS outreach programs, which will maximize their independence and improve their quality of life.
• To assist professionals with meeting the needs of residents diagnosed with MS in long-term care facilities by increasing staff understanding of MS; helping them feel more comfortable with serving people with MS.
• To engage the talents of community volunteers to form a working relationship within the community.
• To create a Friendly Visitor program specifically for those living in LTC facilities and who are homebound.
