Financial Assistance Program

The Oregon Chapter of the National Multiple Sclerosis Society has funding to assist clients with the purchase of durable medical equipment, assistive technology, aids to daily living, in-home care (for respite), home modifications, vehicle modifications, air conditioners, cooling vests, community exercise programs, as well as short-term critical needs, like rent and utilities.

To apply for funding, the Chapter requests that you do the following:
1. Mail or fax the completed Financial Assistance Application to the Chapter.
2. Submit a statement of MS diagnosis from your doctor, and a prescription, if the request is for medical equipment.
3. Provide the necessary bids, the name of your preferred provider and why.

If you have any questions, please contact the Oregon Chapter at 503.223.9511 or 800.344.4867.
