Health Services

Health services are designed to empower people living with MS and help maintain their independence. We offer an array of health services to our members. Click the links below to read the descriptions and requirement.
 

Medical Equipment | Physical/Occupational/Speech Therapy | Counseling Programs | Transportation | Information and Referral Center

 

Medical Equipment

Medical equipment gives members access to a wide range of assistive devices. Insurance companies typically limit the quantity of equipment covered regardless of the range of accessibility and functional need. We may be able to provide up to $250 toward durable medical equipment and $100 towards assistive technology including, but not limited to:

  • wheelchairs
  • scooters
  • grab bars
  • shower chairs
  • Hoyer lifts 
  • reachers
  • Drink Aides
  • sock pulls
  • weighted utensils
  • button fasteners 

For this service:

  • Confirmation of MS Diagnosis from a neurologist
  • Submit a completed Consent to Services form
  • Submit an evaluation from a Physical or Occupational Therapist (the chapter can assist you if you cannot afford one)
  • Insurance and other third-party options must have been maxed out or denied
  • Members may also use this service for the interim while a permanent device is on order
      

Physical/Occupational/Speech Therapy Programs

This service is available for people with MS who will benefit from a therapy to increase their functional independence if insurance and other third-party payment options have been denied. The chapter can also assist with a limited number of co-pays or deductable assistance for ongoing therapy.

For this service:

  • You need a doctor's prescription for the specific type of therapy you are to receive
  • Confirmation of MS Diagnosis from a neurologist
  • Submit a completed Consent to Services form 
     

Counseling Programs

This service is available for people with MS who will benefit from counseling to increase their functional independence if insurance and other third-party payment options have been denied. The chapter can also assist with limited co-pay assistance to pay for counseling.

For this service:

Transportation

In circumstances where no other resources exist, the chapter can assist with round-trip rides to neurology appointments. Please call the chapter for approval a minimum of two (2) weeks prior to the appointment. The chapter can assist with up to two (2) transports per fiscal year.

For this service:

  • Confirmation of MS Diagnosis from a neurologist
  • Submit a completed Consent to Services form
  • Please call to schedule ride two weeks in advance
  • Please include appointment time, date, location and what type of transportation is needed.
     

Information and Referral Center

This cutting-edge center is specifically designed to meet the unique needs of people with MS. Led by professionally trained staff, the Information and Referral Center assists with questions and concerns regarding newly diagnosed patients, general information, pamphlet requests, and referrals to a variety of agencies, organizations, facilities and self-help groups.
 

A Consent to Services form must be completed and submitted to the chapter before any assistance can be provided.
