SELF IMPROVEMENT GRANT PROGRAM
Managing your health is one of the first steps to managing your MS. We congratulate you for having the desire to create a healthier lifestyle. People with MS have often said, "I have MS, but MS does not have me". If you are one of these people, we invite you to JOIN THE MOVEMENT and move towards living a better quality of life.
The Utah State Chapter is committed to healthy living by partnering together with you. We are proud to introduce our new "Self Improvement Grant Program" designed to help fund and encourage overall wellness. Our program compliments Society studies proving aerobic exercise improves both physical and psychological well being of persons with MS. Activities may include things such as taking a yoga or spin class, getting nutritional counseling, or may even assist you with co-payments to your physical therapy appointments. Need ideas? Members of the Utah State Chapter are eligible for one of the following types of grants up to $150 a year.
Term Program Grant - An established term of 90 days maximum
* NMSS will fund 75% of the fee or expenditure, and could include a fitness class, physical therapy, training regimen, piece of exercise equipment, nutritional counseling, etc. For example, you may purchase a yoga pass good for 20 visits costing $100. The Society would reimburse you for $75 once you submit a receipt(s).
On-going Program Grant
* NMSS will fund 50% of the fees for any ongoing wellness program, such as facility memberships, long term exercise programs, etc. For example, you may choose to purchase a gym membership with a payment of $25 per month. After 12 months, you would submit proof of payment (12 x $25 = $300) and be reimbursed $150.
