Care management services are provided through our Moving to Succeed program. We are committed to helping people improve their quality of life anf fulfilling the NMSS vision to one day see a world free of MS. This program serves the following clients:
- Those with chronic long-term care needs that can no longer be met solely by family and friends.
- Clients facing obstacles obtaining needed services due to advanced MS, mental health conditions, complexity of issues, etc.
- Clients at-risk of nursing home placement are a priority.
