Care Management

Care Management Program

The Arizona Chapter of the National MS Society has developed the Care Management Program to promote enhanced quality of life and to help people living with MS to achieve the highest level of independence and autonomy. Our Care Manager works one-on-one with individuals affected by MS, connecting them to the services and resources that are needed to maintain independence.

In order to participate in this voluntary program, a person must have a confirmed MS diagnosis and live in the Phoenix-Metro area. Services may be appropriate if someone living with MS has:

  • Accelerated loss of function
  • Feelings of depression or being overwhelmed
  • A need for more support
  • Difficulty getting the services they need

The services provided by the Care Manager vary depending on each individual’s goal, but may include:

  • Navigation of community services
  • Education
  • Referral
  • Emotional support

Each participant is evaluated individually, with the unique circumstances of the person living with MS taken into consideration. Program availability will be evaluated on a case-by-case basis depending on client needs and Chapter resources. The Care Management Program is growing to include the Northern Arizona area as well as the Tucson area beginning in the Fall of 2012.
