Care management provides for one-on-one professional assistance to help individuals with MS achieve independence and autonomy. For more information contact firstname.lastname@example.org at 760-448-8403. Resources for people living with advanced MS are also available online.
Information cards are available to registered members of the Pacific South Coast Chapter. The emergency cards are the size of a license and display the member's name, physician and emergency contact. The card alerts police, store owners and others of common MS symptoms. To receive a card, please e-mail email@example.com the following information: full name, address, physician's name and phone number and emergency contact name and phone number.
