Physical Therapy

Physical Therapist: Jonathan Goldberg, PT

The physical therapy services are comprehensive and integrated to address the needs of each patient. Each service individually addresses specific needs of the patient. As a team, the rehabilitation is integrated to address the “whole” person. A comprehensive plan is developed and reviewed with the medical director on a weekly basis, to coordinate and meet the needs of each patient, family and physician.

Evaluations are initiated upon referral from a physician. Evaluations include a review of past medical history, patient/family interview and functional assessment, which includes objective tests. Once the assessment is completed, the rehabilitation plan of care is developed and discussed at the Team Conference each week with the medical director.
