Each year the Georgia Chapter hosts its annual MS Service Day. This special day is designated to those clients who are unable to perform certain tasks in or around their home due to their MS or for those households overwhelmed with the impact of MS on their daily life.
On MS Service Day teams of volunteers will be appointed to client homes to complete such tasks as yard work, deep cleaning inside the house and/or garage, interior/exterior painting, organizing closets, moving furniture, packing/storing items in garage or attic, handyman chores, and much more… This year, MS Service Day is Saturday, March 2, 2013!!!
If MS Service Day 2013 sounds like something you, your family, or your household would benefit from and you have not received financial assistance from the Chapter in the last 12 months, please complete the application materials below and return to the Georgia Chapter by October 15, 2012. In order to be considered for participation, you will need to complete and submit the following:
1. Application (pdf)
2. HirEase Disclosure & Consent Form* (pdf)–
3. Letter of confirmed diagnosis of multiple sclerosis from your doctor
*All clients, family members, and volunteers are required to pass a criminal background check prior to being approved. You may print multiple copies of the HirEase form for each adult in the household

We must receive your completed application and all supplemental forms by October 15, 2012. Incomplete applications will not be considered. Clients will be notified by mail of their acceptance in the MS Service Day 2013 program.
