Living Well...Take Charge of Your Health is Maryland's name for the Chronic Disease Self-Management Program (CDSMP), which was designed and evaluated by Stanford University Patient Education Research Center and is now used in many countries.
The Living Well program is offered as a 6 week workshop that is highly structured and intensively interactive for the participants. The weekly sessions last 2.5 hours and are facilitated by trained volunteers who work in two person teams.
