Get the most out of life through Self-Management Workshops
The Maryland Chapter has partnered with several agencies throughout the state of Maryland to offer the Living Well...Take Charge of Your Health program to our members. The program is designed for persons with chronic health conditions or for those who live with persons with chronic health conditions. Living Well...Take Charge of Your Health is Maryland's name for the Chronic Disease Self-Management Program (CDSMP), which was designed and evaluated by Stanford University Patient Education Research Center and is now used in many countries.
The Living Well program is offered as a 6 week workshop that is highly structured and intensively interactive for the participants. The weekly sessions last 2.5 hours and are facilitated by trained volunteers who work in two person teams.
How does a Self-Management Workshop help people with chronic conditions?
The course helps people learn skills to manage their symptoms and get the most out of life. Although there are many chronic conditions, the problems and symptoms associated with these diseases are often similar. Many people with chronic conditions experience: loss of energy, sleeping problems, pain, depression, breathing difficulties and a concern for the future.
Workshops are offered in easily accessible community settings for small groups with 8 to 15 participants.
