Responsibilities:

• To provide support and information services to persons affected by MS, family members, health professionals, and others to access resources and navigate community systems; includes services at an information and referral level, service management level, and care management level.
• Maintain and update Altair database of available resources and new information for MS clients throughout the chapter area.
• Provide support to affiliated clinics by helping clinic staff connect clients to the chapter and helping them access resources, navigate community systems, etc.
• Participate in other projects as assigned.

Qualifications:
Two to four years' experience in human services. BA or BS degree in human services with an ability to apply this education to the understanding of the disease process, community systems and resources. LSW or MSW and license preferred. Strong verbal, written, and interpersonal skills. Ability to assess clients' needs and respond accordingly. Ability to organize and prioritize work; work independently; be a team player and meet deadlines. Must be able to work and interact with a diverse group of individuals.
