The National MS Society, Upper Midwest Chapter can provide limited financial assistance for fees and/or co-pays for counseling to any person with multiple sclerosis who is registered with the chapter.
Funding guidelines
- Assistance is for fees or insurance co-pays for counseling services only. Assistance is not available towards any additional services in conjuncture with the counseling services (i.e. parking, fuel costs, any services prescribed or suggested by therapist, etc.).
- Professionals providing counseling services must be licensed and insured.
- Clients must check to see if the therapist or mental health agency offers a sliding fee scale (discount for services) first as well as investigate any other funding resources before applying for the program. Call 800-582-5296 and press “1” to get assistance finding such resources.
- The program can assist with a maximum of $750 per person, per fiscal year. The fiscal years runs Oct. 1 through Sept. 30 of the following year.
- Clients receiving assistance must reapply for the program each fiscal year if they intend to utilize the program for more than one year.
Application process
- Contact the Upper Midwest Chapter to request an application.
- Complete the application and send it to the National MS Society, 200 12th Ave. S., Minneapolis, MN 55415, along with written confirmation of diagnosis of MS from your physician.
- The application will be reviewed and the applicant will be notified by mail whether the application was approved or denied.
- If approved, the therapist will also be sent a confirmation letter outlining the terms and conditions of the assistance being provided.
