The National MS Society, Upper Midwest Chapter can provide limited financial assistance to help with costs of transportation to medical appointments for any person with multiple sclerosis who’s registered with the chapter.

Funding guidelines
Transportation to medical appointments such as neurology appointments, PT/OT appointments, etc., can be considered for this program. If you have a question about if your transportation request meets the guidelines, please call the chapter.

You must inquire about assistance with the costs PRIOR to the appointment. The chapter will work with you to determine how much assistance you are eligible for based on distance to the provider, etc. Depending on the situation, we’ll work to get a gas card or other assistance to you before your appointment.

You must not be eligible for any other type of transportation assistance (such as Medicaid transportation to appointments) to qualify for this program.

The scholarship can assist with a maximum of $250 per person, per fiscal year. Fiscal years run Oct. 1 through Sept. 30.

Application process
Contact the Upper Midwest Chapter at 800-582-5296 and press “1” to request an application. Complete the application and send it back to the chapter (200 12th Ave. S., Minneapolis, MN 55415) along with written confirmation of MS diagnosis. Application and verification of diagnosis must be on file prior to receiving assistance.

Submitting an application doesn’t guarantee approval. Once your application has been received, a staff member will be in touch with you.
