Once applicants have been offered an internship, the below forms will need to be submitted to the National MS Society at the intern orientation. Students should review these documents and be prepared to fill them out at the orientation:
- Code of Conduct
- Computer User Agreement Form
- Confidentiality/Conflict of Interest Policy and Agreement
- Emergency Contact Information Form
- Intern Waiver
- Stipend Agreement Form
- Personal Information & Stipend Form
If you have any questions, please note that the Volunteer Community Outreach Coordinator will go over these forms with you during the orientation day. More documents will be provided for you to sign during the internship as well.
