Enclose a check for $40 with application (Please make check payable to National MS Society, Gateway Area Chapter) or pay now online. $25 is a tax deductible donation to the National MS Society. $15 covers the cost of the license plates.

When the state approves the MS license plate, we will submit all applications and payment directly to the state.
- In the highly unlikely event that the license plate is not approved, the National MS Society will refund your full $40.
