Independant Contract Worker

The Central North Carolina Chapter of the National MS Society is in need of independent contract social workers for the Reach Out: Services to Maintain Independence Program 

Position:  Independent Contract Worker

 

Contract Social Workers are needed for the following Central North Carolina counties: Alamance, Alleghany, Ashe, Caswell, Davidson, Davie, Forsyth, Guilford, Randolph, Rockingham, Stokes, Surry, Watauga, Wilkes and Yadkin

Qualifications: Must have a minimum of a Bachelor’s degree with 2 years experience providing community based services, preferably Master’s degree with experience in providing community based services. Applicant should be familiar with local community resources. Knowledge of chronic illness and disability issues important. Care management experience preferred.

Knowledge, Skills and Abilities:

  • Strong oral and written communication skills
  • Excellent listening and counseling skills to deal with individuals in crisis
  • Strong interpersonal skills, including a high level of sensitivity toward the needs of people with and affected by multiple sclerosis
  • Ability to function independently, with a minimum of structure and supervision
  • Strong assessment, problem-solving and decision-making skills.
  • Flexibility to work occasional evenings and weekends
  • Ability to travel (every effort will be made so that the care manager will not have to go beyond a 60 mile radius)
  • Valid driver’s license, access to a personal vehicle, proof of vehicle insurance.
  • Knowledge of local resources and/or ability to effectively research local resources

Position Duties:

  • Coordinate and meet with client and family to develop a comprehensive in-home assessment as per program policies.
  • Develop care plan with client and family to address needs identified in home assessment.
  • Provide necessary follow-up visits with client and family as required
  • Link family with needed services within the community for long term interventions
  • Document interactions with the family via progress notes and submit all documentation in a timely manner.
  • Attend required annual trainings as per care manager contract
  • Communicate regularly with Reach Out: SMI ManagerNational MS Society training will be required for all contract service providers prior to assignment. Service providers will also be expected to obtain professional liability insurance prior to service provision and participate in quarterly one-hour conference calls for training and discussion.
