Continuing Care Outreach Program
People with MS in long-term care facilities and those isolated at home deserve the best programs and services available throught the National MS Society, Mid-Atlantic Chapter, as well as avenues to access this information. This was our main goal as we designed this innovative cutting-edge program. This program utilizes an all-volunteer trained advisory council who have researched needs in long-term care facilities. We offer a continuum of care services thereby maximizing independence and improving quality of life for those living with MS.
The program goals are:
- To identify people with MS living in LTC facilities or who are homebound who might benefit from NMSS outreach programs, which will maximize their independence and improve their quality of life.
- To introduce a continuum of free services from the National MS Society, Mid-Atlantic Chapter to residents of long-term care facilities and clients who are homebound.
- To engage the talents of community volunteers to form a working relationship within the community.
- To assist professionals with meeting the needs of residents diagnosed with MS in long-term care facilities by increasing staff understanding of MS; helping them feel more comfortable with serving people with MS.
