Deposit Ticket - Include when you send your donation to our office to ensure that funds are credited correctly
Pledge Form - Keep track of pledges from people who agree to sponsor you
Donation Tracking Sheet - Track your donations!
Receipts for Donors - If donors request a receipt, fill out one of these.
Materials Request Form - If you need materials (brochures, posters, etc.) to promote your walk, request them from our office! Or download and print them yourself from the Promote Your Walk page.
Mile Marker Sponsorship Form - Sell Mile Markers to businesses and organizations for $250 each; an easy way to raise money and get their logo in front of hundreds of Walk MS participants! To see other ways businesses can get involved, check out the Corporate Sponsorship page.
Team Registration Form - Register your team by mailing this form to our office!
