When a person with MS needs transportation, eligibility for assistance must be determined. The individual must be registered with the National Multiple Sclerosis Society, Long Island Chapter, a confirmation of their diagnosis and waiver of responsibility , and an up-to-date financial assistance application must be completed ( within the past 2 years) and on file.
Transportation is provided for the following:
- Medical emergencies
Referrals to other community resources will be made as appropriate to ensure access to quality medical care.
