HOME THERAPEUTIC RECREATION PROGRAM
 

**Are you bored with your daily routine at home?

**Do you feel you could be more active in your home?

**There are alternatives to watching television all day!

Many individuals living with multiple sclerosis find themselves homebound for a variety of reasons. The purpose of the Home Therapeutic Recreation program is to provide individuals living with MS who are isolated in their own home  with the opportunity to analyze how they spend their free time at home and learn skills that enhance the positive use of leisure time in a solitary environment.

A certified therapeutic recreation specialist ( CTRS) will meet with you and assess how you currently spend your time. Together you will work on skills such as time-management strategies, new leisure activities tailored to fit your unique interests and circumstances, an exercise plan and more. The CTRS will introduce you to resources in the community, including chapter-sponsored programs that can reinvigorate your routine and promote a more active lifestyle.

This program requires a referral from your physician; up to five sessions will be offered.
 

ADULT DAY PROGRAM  ( ADP)

Designed to promote participation in social, educational and recreational activities, this 18 session bi-weekly program offers a variety of recreation activities, including arts and crafts, fitness exercises, horticulture, community education lectures and more. Lunch is not provided, so please bring your own to each session. Any additional fees for activities are the member’s responsibility.
Transportation is not provided for this program.

ACCESS the APPLE, a community excursion program


The chapter will host four community based outings per year. You will learn how to “access the apple” and experience NYC firsthand regardless of your mobility status. Trips will be planned to a variety of events:
• theatre
• museums
• sporting events

In signing this release, I understand the intent thereof and will for myself, executors, administrator, and assigns hereby release the National Multiple Sclerosis Society, New York City - Southern New York Chapter, and all sponsoring agencies, organizations and their respective agents and employees from all claims of damages, actions and causes of actions whatsoever, in any manner as a result of my participation in the Recreation and Fitness programs sponsored by the New York City - Southern New York Chapter implemented October, 2012 through June, 2013.

I hereby give the National Multiple Sclerosis Society, NYC-SNY Chapter permission to use my name and any photograph, likeness or image taken of me during the event in any promotional materials, publication or via the web site.

The National Multiple Sclerosis Society, NYC-Southern NY Chapter withholds the right to refuse or dismiss anyone who may cause any disturbance or hindrance in any manner, which could jeopardize the success of the Recreation and Fitness program.

I have discussed my participation with my physician. In the event of an emergency, I hereby give the NYC – Southern NY Chapter permission to provide treatment in case of injury or illness.
