The National MS Society is giving away a Golden Buzzaround Lite Scooter donated by 101 Mobility

Contest deadline: February 19, 2013.
Proof of diagnosis must be submitted by February 19 for the application to be complete. Proof of diagnosis may be uploaded with the application, faxed to 585-442-2817, or emailed to Dee.Mascari@nmss.org (subject line “Scooter Giveaway”).
One person will be chosen by committee to receive the scooter. The winner will be announced during MS Awareness Week, March 11-17, and only the winner will be notified. The winner must arrange with 101 Mobility for delivery or pick up of the scooter before April 1, 2013. If arrangements are not made by April 1, 2013, the committee will award the scooter to an alternate winner.
Scooter recipient is responsible for completing a W-9 form and paying any applicable income tax. A 1099 form will be provided in January 2014.
