Prize Process - how to select & redeem your prize
You qualify for a fundraising prize based on the amount of money you raised. Those walkers raising $250 or more are eligible for a prize.

A Prize Award Form will be mailed to you shortly after your respective Walk MS event along with a return envelope and thank you letter. Next you will need to:

1. Complete the Prize Award Form by indicating what prize you would like based on the contributions you have already turned in. Again, you have until June 30 to keep fundraising to reach your targeted prize level.
2. If you are selecting to join the Mission First Club you will need to check that box next to your prize level and ONLY complete the back of the form if you are purchasing the Mission First Item.
3. Once you have made your prize selection, mail the form back to us in the self-addressed envelope we provided.

For Walkers Selecting To Receive an Online Prize
After completing step 3 from above:

1. A gift certificate will be mailed to you after June 30.
2. The certificate will include instructions and a code for you to be able to redeem the prize of your choice from our online store.

It is important to remember that the Prize Award Form must be returned to us by June 30 along with any additional contributions you have that you would like to go towards your prize level.

Contributions are still accepted after June 30, but will not count towards your overall prize goal. Prizes are not accumulative, and are based on the individuals contributions not the teams totals.

Important Deadlines for 2012

You can continue to fundraise for Walk MS for as long as you would like. To be eligible for a prize and the Top 100 Club, all contributions must be received by June 30, 2012. You must complete and return the Prize Award Form that is sent to you after the event, which indicates what prize selection you have made, by June 30 as well.

All online prize certificates must be redeemed by November 30, 2012. The online prize store will not accept certificates after this date.

Walk MS Prize Structure
Please note this is the prize structure for Walk MS 2011. The deadline to redeem your online certificate from Walk MS 2011 is December 31, 2011. After this date the prize structure will change to reflect Walk MS 2012. If you have any questions, please feel free to contact us at 216-503-4183.
