In addition to our online fundraising tools, the following printable fundraising tools have been created to help with your efforts. Donation Form – If you have donors that do not wish to donate to you online, please provide them with this form so they can mail it in with their check. It’s not necessary for donations to arrive with this form as long as the check includes the Walk Participant's name, team name, and Walk MS location.
Donation Tracking Form – Use this form to keep track of those who donate to you so you can send them a thank you after the walk.
Walk MS Poster – Help us spread the word about all 10 Walk MS events across Colorado. Ask as many store managers as possible to place this poster in their window. Click on this link for a black/white version.
"I'm Walking For" - Show your support and print this "I'm Walking For" bib on orange paper before walk day. Please write who you are participating for and decorate with pictures.
Donation Shoe – Ask as many store managers as possible to help gather donations from their customers for your fundraising efforts. Customers who donate can write their name in the “shoe” and tape it up in the store. *Please print this black/white version on orange paper if possible.
Contribution Receipt – Print and fill this receipt out for donations under $249 dollars.
Cash-In Early Incentive – All Walk MS fundraisers who turn in $50, $250 or $1,000 worth of donations before Thursday, April 30, 2009 will be entered into a drawing for fabulous prizes like a large flat screen TV!

Paper Registration Form - You and your team members have the option to register online through the 'Green Box' at the top right of your screen or you can print this form, complete it for each person being registered and return to us by fax or mail (info located at the bottom of the form).
